Dear Parents/Carers

Public Health England require local authorities to participate in dental epidemiology surveys which help plan and implement services in areas that need it. The local authority then ask the NHS community dental service to come into schools and conduct the surveys.

This year the survey is aimed at children who are 5 years old on a particular date they visit schools, which for The Ridge is the 26th January 2024.  Further details about what is involved is in the attached letter.

As your child fits into this category, a consent form will be sent home today for you to confirm if you are happy for your child to take part in the survey. Please complete the form and return it to school.
